• October 18, 2008 Peoria forms due October 4

• November 2, 2008 Polar Ice Chandler forms due Oct 19

• November 15, 2008 Peoria forms due November 1

• December 7, 2008 Polar Ice Chandlerforms due Dec 23

• December 13, 2008 Peoria forms due November 29

Forms should always be mailed to the hosting club. It does not matter which club you are a member of. Forms should be in the hands of the test chair 2 weeks prior to the test date and to be sure your skater gets a spot you should have them in sooner than that. You must be a current member of USFS to test.
